PRIMARY SERVICES
  • OpenSurvey's service levels are tiered to accommodate small and large organizations alike.
  • Each level is based on the number of forms, submittals (responses) and devices (offline usage) each organization requires to complete research and data management operations.
  • Pricing includes the total monthly service & license fees to OpenSurvey.NET per form, device or submittal, a monthly license to run OpenSurvey on a disconnected smart device (device ID must be provided to validate license), and monthly access to the data synchronization and analytical (reporting) interfaces.
  • Rates begin at $25.00 per month and are tiered with an average cost savings of 5-10% between levels above 5, 10, 25, 50 and and over 50 forms or devices. Enterprise licensing is available for groups of more than 50 forms or devices.
ADDITIONAL SERVICES
  • Form Building Support available at $75 per hour.
  • Custom reports may be incorporated into the online reporting interface for on-demand reporting at the report development and implementation fee (typically between $200 and $500 depending on complexity). After providing specifications against the existing survey, development costs and implementation timeframe will be quoted.
  • Custom Data Export, Formatting, Analysis & Reporting Support available at $95 per hour.
  • Surveying equipment may be leased through WRSG for between $60 - $120 per month per device, depending on equipment configuration and lease timeframe (minimum three month service/equipment lease required).
  • OpenSurvey.NET Service Includes secure online form builder, distribution/embedding options, default results, notification and data management interfaces.
  • The default reporting interface includes prevalence data and tabular format data downloadable in MS-Excel or CSV format.

ADDITIONAL TERMS
  • Potential OpenSurvey.NET Users or Groups may request a demo account by submitting a sign-up request via the OpenSurvey.NET sign-up form.
  • The demo will last 30 days during which the demo account may be extended to a full account by selecting a service level and providing credit card details via the OpenSurvey.NET online service agreement form.  
  • All users and groups will be billed monthly for usage at the maximum level utilized during the previous month.  
  • Discounts are offered for six and twelve month service agreements at elected level paid in advance.  
  • Accounts are automatically renewed at the most recent monthly usage level.  
  • Accounts may be cancelled at any time, however each account will remain active for the remainder of the current billing period with no refunds available once service is billed.
  • Service initiation includes 30 minutes of online training (offered monthly) and first 30 minutes of form building service free.  
  • Registered users have access to OpenSurvey's 'Problems & Solutions Knowledgebase' and the online help and service interfaces.
